Age Guidelines: Your child (ren) MUST turn 3 before November 1 of current school year AND MUST NOT turn 5 prior to November 1 of current school year, to be eligible for Head Start

Documentation to bring with you (or have available for phone apps):

• Name and date of birth of ALL household members
• Home Address
• Daycare Address
• Amount of Annual Income (current or previous calendar year) (Please click here for Federal income guidelines, to see if you qualify)

Age Guidelines:  Early Head Start serves pregnant women and children through the age of 3.

If you’re application has been accepted, a Family Advocate will contact you to schedule an enrollment appointment.  Please bring the following documentation along with you to your appointment (The following information pertains to the child being enrolled and the parents/guardians):

• Proof of Income
• Social Security numbers
• Immunization record of child
• Child’s latest physical (MUST be within the past 12 months)
• Verification of birth of child (Birth Certificate or Hospital Certificate)
• Dental exam performed by a licensed dentist MUST be completed within first 90 days of child’s enrollment
